What to bring to your first appointment
Try to arrive a few minutes early for your first appointment to allow yourself time to complete some forms. To make things go faster, please bring with you:
- Your current doctor’s name and contact info
- A list of your current prescriptions
- Any information on allergies or existing medical conditions
- Your dental health insurance booklet
Financial policy
Payment is due at the time of treatment. Please speak to our front desk if you require assistance with payments. We accept payment by cash, debit, or credit card.
Insurance
We provide direct billing with most insurers. Please note that you are responsible for any uninsured costs. Our front desk team can help you understand your coverage and treatment plan.
